Serial # 095s6962 (Or the first could be C, the s could be a 6.)
Part # 591558-1
Radio Appliance: 3478
Date: ?

Thrift stores sell them for around $25.
Maybe $40, if it's got a mint condition Elvis Presley record stuck in the storage chamber.
